Your First Visit We
welcome new patients. A patient registration form is mailed prior to your first appointment so that you can fill it out ahead of time, at your convenience, and bring it with you at the time of your appointment. We ask that all new patients arrive at least 15 minutes early to allow ample time to complete registration and related paperwork. Please bring your insurance card, Medicare card, Medicaid form, General Assistance form, and/or any required HMO or insurance referral forms to your first visit. It is a good idea to bring these materials to all physician or hospital appointments, in case questions arise. In
the future, if your address, telephone, insurance coverage or identification
numbers change for any reason, please notify our office during your
next appointment. Periodically, our staff will verify your current address
and related information to make sure that our records are accurate. Appointments We try to see all appointments at their properly scheduled time. We ask that you arrive for your appointment on time, or notify us if there will be a delay. There may be times when an emergency case at the hospital or the emergency room may cause the schedule to be unavoidably delayed. If this is the case, you will be notified and given the opportunity to reschedule your appointment. For

and Payment Information We are participating providers for Medicare, Medicaid, Blue Cross/Blue Shield, and Priority Health HMO. We normally do not bill insurance carriers for office visits, with the exception of Medicare, Medicaid, Blue Cross, and Priority Health HMO. If you have commercial insurance, we will file a courtesy claim to your insurance company for office service if payment is made in full at the time of service. We will provide you with a copy of your "superbill" to submit to your insurance company to facilitate your reimbursement. All surgeries (except vasectomies) and hospital services are billed directly to your insurance carrier. Each insurance company sets its own fee schedule for reimbursement. Please remember that your coverage is a contract between you, your employer, and your insurance company. Our services are a contract between you and Bay Area Urology Associates, P.C. Our fees are set with much consideration and reflect a fair charge for professional services. At times, there will be services or fees that will not be completely covered by your insurance company. All account balances are the responsibility of the patient from the date of service. We
are happy to assist in providing fee schedules in advance and to assist
in making financial arrangements with you. Please ask for assistance
